Useful Reports
Achievement and Awards (Club)
The Achievement and Awards (A & A) report gives you credit for things you are probably already doing. The winners banner patch is one of the largest.
Clubs are encouraged to submit this form quarterly, at the end of each quarter. The District A & A Chair, Lorraine Coots, will tabulate the results and the winners will be announced at the 1st Quarter Conference next October.

Club Roster Adjustment (Club)
We use this form to add new members and delete members who are leaving us. Until just a few years ago, we had to use paper and postal mail (to several people) for this form. Now, Optimist International has provided us a wonderful tool for instant Roster Adjustments.

Membership Application (Club, Zone and District)
You just never know when you might need one of these. I suggest you print some now and keep them handy. They includes places for very useful information about your new members.

Download Information
District Directory Request (Club, Zone and District)
The District Directory is full of valuable information about District members along with Club officers. You can print out the District Directory yourself.
For security reasons, each request is approved separately by the District Directory Chair, Michael T Gray before we email you the file. This keeps our junk mail down.
PNW District Policies (Club, Zone and District)
The Pacific Northwest District policies were updated at the 2008 convention (at Chilliwack). If you have any questions, you can contact our Parliamentarian, James Slauson.
PNW District Strategic Plan (District)
This document sets our long term goal for the district and should provide an even flow for each fiscal administration.
Note from the Webmaster
Many of these forms are in PDF format. They require Acrobat Reader to be read properly. The program is free for the reader, but costs if you want to write.
